Appearance: Well, here's his only appearance in canon:

Unfortunately, it's a hazy imprint of a memory from over 20 years ago. So of course he doesn't actually look like that! It's unknown as to what his actual appearance was, and it should be taken into account that Hawke (the main character and Malcolm's eldest child) is customizable. For a younger Malcolm, I created a custom Dragon Age 2 MC to give an idea of how I imagine he'd look, based loosely off the default appearance for both male and female Hawke.

History: Being a minor NPC who dies before the events of Dragon Age 2, what is known about Malcolm's past is a bit vague at best. And considering Bioware's knack for retconning left and right...a bit of headcanon is needed to put all of these in order.
Malcolm's place of birth is currently unknown, but it's often assumed to be Ferelden, as Hawke's uncle describes Malcolm as a "Fereldan apostate." Being a mage, he was taken from his family at a young age and raised in the Circle, most likely the one at Lake Calenhad. Every mage within the Circle is required to take the Harrowing in order to continue...well, living; refusal to take the Harrowing means you will either be killed or turned Tranquil, which means having your connection cut off from the fade, severing not only your magic, but all of your emotions. It's most likely that Malcolm stayed in the Circle long enough to pass his Harrowing, considering dialogue in the Legacy DLC, but at some point he actually leaves the Circle. He was able to escape with the aid of a templar named Maurevar Carver, who believed that Malcolm could do more good out in the world than trapped in a tower for the rest of his life. Malcolm respected the man a great deal, and would go on to name one of his children Carver. He vowed to only let his magic serve the best in him, and not give in to his baser instincts.
On the run from the templars, Malcolm managed to hide by using martial skills to pose as a mercenary. He drifted in and out of different mercenary groups, but eventually his work with the Crimson Oars sent him to Kirkwall, which was actually a bit dangerous, as Kirkwall is the seat of Chantry power in that region. It's here in Kirkwall that he met Leandra Amell, a young noblewoman, and began courting her. There's some conflicting stories as to how the two actually met; according to some game dialogue, Malcolm was a junior enchanter in the Circle at Kirkwall, but this contradicts codex entries that state that he had already left the Circle and was doing mercenary work in Kirkwall. However, eventually Malcolm is discovered to be a mage when he uses his magic to save the life of the Crimson Oars' leader. Forced to leave the city, he makes a bold move to say goodbye to Leandra, sneaking into a masquerade ball being held for the visiting Orlesian empress. Leandra decided to leave with Malcolm, and the two fled Kirkwall. I believe that at this point Leandra reveals to Malcolm that she's pregnant with their firstborn, but it hasn't been confirmed by canon.
Of course, their escape hit a snag, but one that nobody expected. The Grey Wardens were in charge of a magical prison located in the Free Marches near Kirkwall, and in order to keep the powerful magister sealed inside, they needed the blood of a non-Warden mage. Most likely they became aware of Malcolm when his cover was blown and the templars began to search for him, and during their escape they actually kidnapped Leandra and used her to blackmail Malcolm into renewing the seals in the prison. Forced to use blood magic (a very dangerous and forbidden type of magic in most of Thedas, which can lead to being possessed by a demon), he remarks that he hopes that the child will not be born with magic. Eventually he's reunited with Leandra and the two flee the Marches and head for Ferelden. They have two more children, twins named Bethany and Carver, and move around to avoid templar detection before finally settling down in a small village called Lothering in 9:20 Dragon. Here is where Malcolm spends his last years, passing away from an unknown illness in 9:27 Dragon, three years before the start of Dragon Age 2.
Timeline: I will be taking Malcolm from his time in Kirkwall, where he has begun courting Leandra but is still in hiding as a mercenary, around 25 years before Dragon Age 2.
